Instructions: Please fill out this online form in its entirety, submit your video file, and then click on the "Submit Video and File" button at the bottom of this page. Please make sure that your video clips are less than 500 MB and 3 minutes or less. Please remove any patient identifying information. Video clips should be formatted as .mp4 or .mov files.
